Thank you for participating 
in the 2nd Annual
“O Holy Night” 
Nativity Exhibit (2011)



Register your nativity
  1. Print out 1 Nativity Registration Form for each nativity you wish to donate (up to 5 nativities per family, no limit on decorations). 
  2. Label each piece with your first and last name.
    • If the piece is too small use initials. 
    • Pieces without labels will not be accepted. 
    • Labeling each piece is critical so we can return all pieces correctly.
  3. Bring your completed registration forms with all nativity items, labeled with your name to the Chruch (7014 Frankford Avenue) at the drop off times indicated below.

Register your decorations: Check HERE for a list of needed decorations.
  1. Print out 1 Decorations Registration Form for all decorations you wish to donate (you may use just one form for all non-nativity decorations).
  2. Label your decorations with your first and last name. Use permanent marker or tape that won't come off.
  3. Bring your completed registration forms with all nativity and decoration pieces, labeled with your name to the Chruch (7014 Frankford Avenue) at the drop off times indicated below:



CHECK IN (LDS church, 7014 Frankford Avenue, Lubbock)  
* Please drop off on Monday if you can so we can start working on the set up.
          Monday, November 28, from 12 – 5              
          Tuesday, November 29, from 5 - 7

PICK UP
           Monday, December 5, from 9 - 1
